CenterWell Home Health is seeking a dedicated Occupational Therapist to provide in-home rehabilitation, helping patients regain strength, mobility and independence in their own homes.

Qualifications

• Current and unrestricted OT licensure.
• Current CPR certification.
• Valid driver's license, auto insurance and reliable transportation.

Responsibilities

• Assess patient’s daily living and work-related skills and develop retraining programs.
• Confer with physicians and health care team to update the Plan of Care.
• Provide therapeutic treatment to improve/restore strength and range of motion.
• Document patient care observations and progress accurately in day services.
