St. Elizabeth Home Health is seeking a skilled Occupational Therapist to join our home health team on a PRN basis. You will assess patients in their homes, develop and implement individualized plans, and guide patients toward independence and improved quality of life.

Responsibilities include teaching safety, documenting progress, and coordinating with physicians and other care providers. A current OT license and CPR/BLS certification are required.

Competenze

• Bachelor's degree from an accredited school approved by the American Occupational Therapy Association.
• Active OT license in the state where you practice.
• Must obtain and maintain current CPR/BLS Certification.

Mansioni

• Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans.
• Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
• Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
• Monitor patients progress and adjust treatment plans as necessary to meet changing needs; use outcome-based measures to assess effectiveness.
• Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.
