Job summary

BAYADA Home Health Care in Bloomsburg is seeking an experienced Occupational Therapist (OT) for per diem or part-time home health visits across Columbia County to serve adult and geriatric patients in their homes. You will perform comprehensive evaluations, develop individualized intervention plans, assess home safety, and educate clients and families.

Documentation is completed using state-of-the-art EMR tools as part of a multidisciplinary care team.

Qualifications

• PA OT license is required.
• Minimum one year of OT experience in a healthcare setting.
• Graduation from an accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal, organizational, and problem-solving skills.
• Experience with EMR preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform comprehensive evaluations of functional status and ADL/IADL performance.
• Develop individualized intervention plans to achieve outcomes.
• Assess home safety and environmental adaptations.
• Educate patients and caregivers on rehabilitative care and independent living.
• Document observations and interventions using an EMR-enabled device.
