Job summary

Interim HealthCare of Queen Creek is seeking a Home Health Occupational Therapist (OT) for part-time coverage across Queen Creek and surrounding East Valley communities. The role focuses on evaluating home environments and enabling patients to restore daily living activities with individualized therapy plans.

As an OT, you will work independently with one-on-one patient care, coordinate with nursing and other therapists, maintain accurate documentation, and ensure goals are met in patients’

Qualifications

• Graduate of an accredited occupational therapy program.
• Active Arizona OT license is required.
• Current CPR certification.

Responsibilities

• Evaluate functional abilities, routines, safety risks, and barriers within the home.
• Develop and update individualized therapy plans aligned with physician orders and patient goals.
• Provide interventions that improve strength, coordination, endurance, cognition, and daily living skills.
• Teach adaptive techniques for bathing, dressing, grooming, meal preparation, and other activities.
• Recommend adaptive equipment and environmental modifications to support safety and independence.
• Educate patients and caregivers on exercises, strategies, equipment, and the plan of care.
• Track progress, adjust interventions, and maintain timely, accurate documentation.
• Collaborate with nursing, physical therapy, speech therapy, and other care team members.

What You Bring

• Graduate of an accredited occupational therapy program with an active Arizona OT license
