BAYADA Home Health Care, Inc. in Reisterstown, MD, is seeking a Home Health Occupational Therapist to perform full-time visits for our Baltimore Visits office in Towson. The role focuses on rehabilitative care for adults and geriatrics, delivering 1:1 home visits across Baltimore County.

Responsibilities include patient evaluations, adaptive planning, home environment assessments, and caregiver education. MD OT licensure and one year of OT experience preferred; EMR skills are beneficial.

Qualifications

• Maryland OT license required.
• Minimum one year of OT experience.
• Graduate of an accredited OT program.
• Ability to work independently and manage time.
• Strong interpersonal and organizational skills.
• Prior EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough functional status evaluations and assess ADL/IADL performance.
• Develop individualized intervention plans to improve outcomes.
• Evaluate home environments for hazards and independence barriers.
• Educate patients and family members on rehabilitative care.
• Document observations and interventions accurately in EMR.
