Job summary

UnitedHealth Group is seeking an Occupational Therapist for Home Health in Florida. You will assess and evaluate patient care needs related to functional status, daily living activities, fine motor skills, home assessments and adaptive equipment, following the plan of care and coordinating with the care team.

Responsibilities include evaluating functional status, CMS documentation, and collaborating with physicians to develop and adjust therapy plans.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides service within the scope of practice as defined by state laws, in accordance with the plan of care and coordination with health-care team
• Evaluates patient functional status and documentation for CMS; consults with physician to develop care plan
• Observes, records, and reports patient response and changes to condition
• Conducts patient assessments evaluating functional level using diagnostic tests
• Ability to work with diverse patient populations in home settings
