Job summary

Optum is seeking an Occupational Therapist (OT, OTR/L) for Home Health in Louisiana HomeCare. The role supports assessment, ADLs, home assessments, and adaptive equipment within CMS guidelines, collaborating with a multidisciplinary team.

Responsibilities include evaluating functional status, documenting CMS-relevant data, and communicating patient progress to the care team. Flexible schedules are available across full-time, part-time, and PRN deployments.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to function in any home situation regardless of client characteristics.

Responsibilities

• Provides service within the scope of practice as defined by state OT laws, in accordance with the plan of care and in coordination with the health care team.
• Evaluates the patient's functional status and CMS-related documentation; consults with physician on therapy plan of care.
• Observes, records, and reports patient response to treatment and condition changes to nurse supervisor and physician.
• Conducts patient assessments evaluating functional ability using diagnostic/prognostic tests.
