Qualifications

• Current Occupational Therapy licensure in state of practice.
• CPR certification must be current.
• Valid driver’s license and access to dependable transportation or public transit.

Responsibilities

• Assess and evaluate patient care needs related to functional status and daily living activities.
• Develop and coordinate therapy plans of care with the health care team.
• Document patient progress, respond to changes, and communicate with nurses and physicians.
• Conduct home assessments and recommend adaptive equipment as needed.
