Job summary

LiveWell Homecare Agency, LLC seeks an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions per physician-directed home health plans. You will collaborate with interdisciplinary teams, assess mobility, strength, and function, and develop care plans with measurable goals.

Responsibilities include documenting progress, modifying treatment plans, discharge planning, educating patients and caregivers, and occasionally supervising other staff.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid CPR certification.

Responsibilities

• Conduct assessments and interpret tests to establish goals.
• Deliver therapy services per physician orders.
• Maintain accurate documentation and modify care plans as needed.
• Coordinate with healthcare team and educate patients/caregivers.
• Supervise other staff as applicable.
