Job summary

Patriot At Home is seeking a dedicated Occupational Therapist to provide home-based OT services within a 30-mile radius of Farfield, OH. The role involves collaborating with physicians, performing initial and ongoing assessments (including OASIS), and guiding families in the OT program at home.

The ideal candidate is a licensed OT with at least one year of institutional experience and a current CPR (BLS) certification.

Qualifications

• Graduate of an OT program approved by the AMA and AOTA.
• Preferably member of the American Occupational Therapy Association (AOTA).
• State licensure required.
• Minimum one year of institutional experience.
• Criminal background check required.
• Current CPR (BLS) certification.

Responsibilities

• Assist the physician in evaluating the patient's level of function by applying diagnostic and prognostic procedures.
• Conduct initial and ongoing comprehensive OT assessments, including OASIS.
• Ensure that physician orders are appropriate and discuss necessary changes.
• Teach, supervise, and counsel the family and patient in the total Occupational Therapy program at home.
• Communicate with the physician regularly regarding patient's status and orders.
