Job summary

Patriot Homecare is seeking a licensed Physical Therapist to provide comprehensive home health assessments and individualized treatment plans in Trumbull County. You will collaborate with physicians and the care team to implement therapy goals and educate patients and families.

The role requires OASIS proficiency, at least one year of home health experience, and CPR/BLS certification. Flexible scheduling and supportive benefits accompany competitive compensation.

Qualifications

• Graduate of a Physical Therapy school approved by the AMA in collaboration with the APTA.
• Licensed or registered by the State.
• At least one year of institutional/home health experience.
• Criminal background check required.
• Current CPR (BLS) certification.

Responsibilities

• Provide initial and ongoing comprehensive assessments of the patient's needs, including OASIS assessments.
• Establish a treatment plan in consultation with the physician and revise it with physician approval as necessary.
• Assist in development and implementation of the interdisciplinary patient care plan as it pertains to Physical Therapy.
• Assist the physician evaluating patients by applying diagnostic and prognostic muscle, nerve, joint, and functional ability tests.
• Teach, supervise, and counsel the family and patient in the physical therapy program.
• Write clinical/progress notes on the date of the visit and incorporate in the patient’s record within 72 hours.
