Job summary

Graham Healthcare Group in Bloomingdale, IL seeks a Home Health Occupational Therapist to join our dedicated care team. This full-time, staff position offers $80K-$100K per year, with a focus on delivering patient-centered OT services at home.

You’ll collaborate with a multidisciplinary team, manage and deliver care plans, drive to patient locations, and work in a flexible schedule while earning strong benefits and growth opportunities.

Qualifications

• Bachelor or Master’s degree in Occupational Therapy from an accredited program.
• Licensed Occupational Therapist in the state of Illinois.
• Minimum of one year of healthcare experience.
• Must have a valid driver’s license and be able to travel ~20% of the day.
• Ability to meet charting deadlines and manage time effectively.
• Experience using tablets, mobile phones and EMR software.

• Provide management and delivery of patient care plans.
