Job summary

Addus Home Health is seeking a dedicated Occupational Therapist to provide professional home-based care. You will work with clients in their residences to implement the plan of care, guide families, document progress, and coordinate with care teams to enhance independence and function.

Qualifications

• Graduate of an accredited occupational therapy curriculum.
• Certified as an occupational therapist by AOTA, American Occupational Therapy Association.
• Current Occupational Therapist license in the state in which practicing.
• Minimum one year of work experience as an Occupational Therapist.

Responsibilities

• Guide and instruct the client in prescribing therapeutic self-care and create activities that are directed toward improving independence and physical and cognitive functioning.
• Participate in the development of the plan of care.
• Prepare a diagnostic evaluation report of each client and make recommendations.
• Prepare clinical and progress notes.
• Advise, consult and teach appropriate treatment procedures and supportive activities to clients, family member and other agency staff.
• Establish household management routines.
• Design and/or order appropriate adaptive/assistive devices and/or splints to promote optimal level of functioning.
• Instruct clients and their families in activities of daily living.
• Attend care conferences as deemed necessary.
• Instruct the family in the client's total occupational therapy program.
