Personal Touch Home Health Care Services Inc. is seeking an Occupational Therapist to provide one-on-one, in-home therapy, offering schedule flexibility and meaningful patient impact.

The role involves assessing mobility, strength, and functional levels, developing personalized care plans, and delivering therapy per physician orders in patients’ homes. Collaboration with families and the care team is essential to ensure optimal outcomes and a high quality of life for clients.

Qualifications

• Degree in Occupational Therapy from an accredited program.
• Valid, unrestricted state license (e.g., WI, MN).
• Current CPR certification and valid driver’s license.

Responsibilities

• Conduct initial and ongoing assessments of mobility, strength, and functional levels.
• Administer and interpret tests to set functional goals and outcomes.
• Develop and document personalized evaluation and care plans, adjusting as needed.
• Deliver occupational therapy services according to physician orders.
• Maintain accurate documentation ensuring compliance and facilitating effective discharge planning.
• Collaborate with healthcare team members, patients, and caregivers for coordinated care.
• Supervise other personnel (e.g., OTA, HHA) as applicable.
• Complete OASIS assessments where permitted by state regulations.
