Affinity Home Care is seeking compassionate, motivated Occupational Therapists to join our growing Home Health team serving Philadelphia and surrounding counties.

As an OT, you will evaluate patients in their homes, develop individualized treatment plans, and help clients regain independence in daily activities while ensuring safety and quality of life. We value clinical autonomy, collaborative care, and flexible scheduling for work-life balance.

Qualifications

• Active Pennsylvania OT license required.
• Graduate of an accredited OT program.
• Home health experience preferred.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.

Responsibilities

• Perform comprehensive OT evaluations in patients' homes.
• Develop individualized treatment plans based on patient goals.
• Provide skilled OT services in home settings.
• Improve ADLs and IADLs for independence and safety.
• Educate patients and caregivers on techniques and safety.
