Job summary

Aveanna Healthcare seeks an Occupational Therapist to join our home health team in Wisconsin. You will assess, teach, and deliver therapy interventions directed by the physician within the home health plan of care, collaborating with an interdisciplinary team to optimize patient well-being.

The role requires completion of an OT degree, an active state license, CPR, and a valid driver’s license. Schedule is Mon-Fri, full-time with flexible hours, serving Bloomer, Chetek and Chippewa Falls

Qualifications

• Must have an OT degree and active state license in Wisconsin.
• Current CPR certification.
• Valid driver’s license and proof of automobile insurance.

Responsibilities

• Perform initial and ongoing assessments of mobility, coordination, strength, and current level of function.
• Provide a written evaluation and plan of care defining OT needs, goals, progress and expected outcomes.
• Delivery of occupational therapy services to comply with prescribed physician orders.
• Maintains plan of care through timely documentation, adjusting goals and treatment as needed, and determining discharge plans.
• Discharge planning and educating patient and caregivers on home exercise program.
