Classic Home Care, a nurse-owned and operated skilled home health agency serving Southeast Michigan since 2003, is seeking an Occupational Therapist to join our home health team.

This contingent, patient-centered role involves providing skilled OT services in patients' homes, collaborating with RNs and case managers, and contributing to goal-oriented care plans. Michigan licensure and reliable transportation are required.

Qualifications

• Active Michigan Occupational Therapy license.
• Home health care experience preferred.

Responsibilities

• Provides skilled occupational therapy in accordance with the physician’s plan of care.
• Assists in evaluation and assessment of patient functioning and plan of care.
• Designs orthotics as indicated by patient condition.
• Treats patients to restore function and self-care activities.
• Observes and reports patient reactions and barriers to outcomes.
