Job summary

GrandCare Health Services seeks an Occupational Therapist to provide high-quality home-based OT services in Southern California. You will work with patients in their homes, collaborate with physicians and caregivers, and help patients achieve functional recovery after surgery or illness.

The role offers autonomy in clinical practice, a supportive interprofessional team, and opportunities to specialize, with compensation based on per-visit visits and potential bonuses.

Qualifications

• Must hold a valid California Occupational Therapist license.
• Current CPR certification required.
• Minimum 2 years of clinical OT experience.
• Must have a valid driver's license and reliable vehicle.

Responsibilities

• Provide patient, caregiver, and family education to support recovery and independence.
• Communicate with physicians and care team regarding patient progress and plans.
• Evaluate functional status, strength, endurance, and activities of daily living.
• Assess home environment for safety and independence.
• Recommend adaptive equipment, splints, and orthotics as needed.
• Develop and monitor individualized treatment plans with measurable goals.
• Teach patients to regain daily living skills after illness or surgery.
• Maintain accurate clinical documentation.
