Job summary

BAYADA Home Health Care in Alburtis, PA is seeking a Home Health Occupational Therapist to provide compassionate, 1:1 OT visits in clients' homes across Lehigh County. This per visit opportunity supports adult and geriatric patients with rehab goals, while offering flexible schedules—from per diem to full-time.

As part of a multidisciplinary team, you will evaluate function, develop treatment plans, educate families, and document care with EMR.

Qualifications

• Current Pennsylvania Occupational Therapist license.
• Graduation from an occupational therapy program accredited jointly by relevant bodies.
• Ability to work independently and manage time effectively.
• Strong interpersonal and organizational skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients in designated geographic territories.
• Perform thorough evaluations of patients’ functional status and ADL/IADL performance.
• Develop individualized intervention plans to achieve favorable outcomes.
• Evaluate home environment for hazards and barriers to independence.
• Educate patients and caregivers in rehabilitative care and activities.
• Document observations and interventions using EMR.
