Job summary

Amedisys Home Health is seeking an Occupational Therapist to deliver comprehensive OT services under a physician-approved plan of care in a home health setting. You will assess patient needs, document findings, and ensure safe, effective care at the patient’s home.

Required are an ACOTE-accredited OT program, active state OT license, and NBCOT eligibility. A strong focus on risk management, patient safety, and collaboration with care teams is essential.

Qualifications

• Current license to practice occupational therapy in the assigned state.
• Graduate of an OT program accredited by ACOTE/AOTA or successor.
• Eligible to take or has completed NBCOT certification examination.

Responsibilities

• Provide OT services under physician-approved plan of care.
• Ensure plan of care addresses identified problems.
• Ensure safety and effectiveness of OT delivery.
• Complete documentation timely and accurately.
• Supervise home health aide when RN is not active in the case.
• Other duties as assigned.
