Job summary

Peace and Harmony Homecare LLC is seeking a licensed Occupational Therapist to provide home-based OT services in collaboration with primary care physicians and the health care team. The OT will conduct evaluations, develop plans of care, and educate patients and families.

The role requires MA licensure, CPR certification, a criminal background check, and prior experience in home health settings is preferred. The OT works with RN case managers to deliver essential services in patients' homes.

Qualifications

• Licensed occupational therapist in MA.
• ACOTE-accredited OT education program completion or equivalent.
• Eligible for or completed NBOT entry-level certification.
• Minimum of 1 year experience as an OT, preferably home care.
• Criminal background check and current CPR certification.

Responsibilities

• Receives intake referral information and prepares OT paperwork/tools for visits.
• Performs home safety checks and environmental assessments.
• Conducts OT evaluations or skilled visits and documents accordingly.
• Develops and submits plan of care (POC) to physician for approval.
• Evaluates and assists with ADL activities and functional levels.
• Collaborates with RN case managers, physicians, and other team members.
