Job summary

UnitedHealth Group in the United States is seeking an Occupational Therapist for Home Health to assess and evaluate patient care needs related to functional status, activities of daily living, and home safety, including adaptive equipment and CMS documentation.

You will collaborate with the care team, complete patient assessments, and contribute to a compassionate, compliant care plan while delivering high-quality service to diverse patients in home settings.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification or ability to complete within 90 days of hire.
• Ability to manage multiple tasks and work independently with good communication and organization.

Responsibilities

• Provides service within the scope of practice as defined by state laws and plan of care with the health care team.
• Evaluates functional status and CMS documentation needs, consulting with the physician on the care plan.
• Observes, records, and reports patient responses and condition changes to supervising staff.
• Conducts patient assessments using functional ability tests and records findings.
• Ability to work in home settings regardless of demographics and conditions.
