Job summary

Home Health Care, Inc. is seeking an Occupational Therapist (OTR/L) to provide home health rehabilitation in Albert Lea, MN. This full-time role offers a schedule Monday–Friday with compensation ranging from $80,000 to $150,000 annually.

You will evaluate patients in their homes, develop personalized plans, and work with an interdisciplinary team to promote independence. The position emphasizes autonomy, quality of care, and flexible scheduling, with支持 continuing education and licensure

Qualifications

• Active Minnesota Occupational Therapist (OTR/L) license.
• Strong clinical reasoning and patient-centered approach.
• Excellent communication and organizational skills.
• Ability to work independently while managing a field-based schedule.
• Home health experience welcomed but not required.
• Valid driver’s license and reliable transportation.

Responsibilities

• Complete comprehensive occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans focused on ADLs, safety, upper extremity function, cognition, and independence.
• Recommend adaptive equipment, home modifications, and energy conservation strategies.
• Educate patients and caregivers to promote long-term success and reduce fall risk.
• Collaborate with physicians, nurses, physical therapists, speech-language pathologists, and the interdisciplinary care team.
• Maintain accurate and timely documentation using our electronic medical record.
