Job summary

Enhabit Home Health & Hospice is seeking an Occupational Therapist to help patients regain independence in their homes through personalized therapy plans and evidence-based interventions. You will assess daily living skills, document progress, and adjust treatments while collaborating with a multidisciplinary team to ensure high-quality home health services and patient-centered care.

Licensed OT in the state, with Medicare home health experience preferred and a commitment to ongoing professional

Qualifications

• Must be a graduate of an approved school of Occupational Therapy and licensed as an Occupational Therapist in the state of practice.
• A minimum of one-year experience is preferred, with potential exceptions approved based on prior leadership exposure.
• Medicare home health experience is preferred.

Responsibilities

• Help patients in the home improve daily living skills and functional abilities through targeted occupational therapy.
• Develop individualized therapy plans, document progress, and adjust interventions as needed.
• Collaborate with a multidisciplinary team to support safe, effective home-based care.
