AdvisaCare is seeking a compassionate Home Health or Hospice RN to serve Traverse City, Kalkaska, Baldwin, Cadillac, Manistee, Gaylord, and surrounding areas. You will assess patients, implement plans of care, and educate families while collaborating with a care team.

Requirements include an active Michigan RN license, at least 1 year in home health or hospice, and strong communication skills. Join AdvisaCare for meaningful, patient-centered service with supportive colleagues.

Qualifications

• Current Michigan RN license in good standing.
• Minimum 1 year of home health and hospice experience.
• Empathetic approach to patients and caregivers.
• Willingness to participate in field visits and meetings.

Responsabilités

• Assess patient’s physical, functional, psychosocial and environmental needs.
• Implement and revise the individualized plan of care.
• Ensure compliance with AdvisaCare policies and Medicare requirements.
• Educate patients and families on disease processes, self-care, end‑of‑life care and ethical concerns.
• Supervise paraprofessionals delivering services.
• Prepare clinical and progress notes and participate in IDG conferences.
• Participate in Home Health and Hospice performance improvement programs.
• Maintain high levels of customer satisfaction with clients, families and caregivers.
• Communicate information using current processes and technology within the organization.
