Job summary

AdvisaCare seeks compassionate Home Health or Hospice Registered Nurses to serve communities in Michigan. You’ll provide one‑on‑one patient care, assess needs, and implement care plans in a supportive, team‑oriented environment.

Role involves field visits, adherence to policies, and educating patients and families about disease processes and self‑care. Flexible full‑ and part‑time opportunities across Traverse City, Kalkaska, Baldwin, Cadillac, Manistee, Gaylord and nearby areas.

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year home health and hospice experience.
• Empathetic approach to patients and caregivers.
• Participate in field visits daily, weekly office meetings as indicated.
• Ability to adapt to patient’s status and needs.

Responsibilities

• Initial and ongoing assessment of patient’s physical, functional, psychosocial and environmental needs.
• Implement the individualized plan of care and revisions as needed.
• Compliance with AdvisaCare policies and Medicare requirements.
• Consult with patient/family on disease process, self-care techniques, end‑of‑life care, and ethical concerns.
• Supervision of paraprofessionals providing services to patient.
• Preparing clinical and progress notes and participating in IDG conferences.
• Participate in Home Health and Hospice performance improvement programs.
• Maintain high level of customer satisfaction with clients, families and caregivers.
• Communication of information using current processes and technology within the organization.
