Job summary

AdvisaCare Hospice in Michigan is seeking a flexible Registered Nurse to provide compassionate, one-on-one home hospice care. You will support patients and families with dignity, comfort, and respect, spending meaningful time where it matters most.

You’ll collaborate with an interdisciplinary team, supervise paraprofessionals, and contribute to patient/family plans of care. This role offers on-call rotation, a 30–40 hour weekly schedule, and a benefits package designed to acknowledge your

Qualifications

• Current Michigan license in good standing.
• Minimum 1 year hospice experience.
• Empathetic approach to patients and caregivers.
• Field visits daily and weekly office meetings as indicated.
• Ability to adapt to patient needs on a full-time on-call basis.

As a Hospice RN with AdvisaCare Hospice you'll provide emotional support and compassionate clinical care, helping patients experience their final days with dignity, comfort, and respect.

You’ll also:

• Conduct appropriate supervisory visits
• Provide clinical observation and feedback to assigned team members
• Participate in interdisciplinary group meetings
• Help develop, review, and revise patient/family plans of care
• Advocate for the needs and wishes of patients and their families

Requirements

• Initial and on-going assessment of patient's physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervision of paraprofessionals providing services to patient
• Preparing clinical and progress notes, participating in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization

Requirements:

• Current Michigan License in good standing
