Cook Children's Health Care System is seeking an Admin on-call Coordinator for Home Health in Fort Worth. The role supports open communication with patients, physicians, staff and referral sources, following up on unresolved office-hour issues and coordinating new patient starts.

Graduate of an accredited school of Nursing with 2-3 years in home health or related settings is preferred; pediatric experience is a plus. Current Texas license and CPR are required, with CRNI/CPN preferred.

Qualifications

• Open communication with clients, physicians, staff and referral sources.
• Follow up on unresolved issues from office hours (lab results).
• Coordinate new patient starts as required.
• Document communications, referrals and problems timely.
• Provide office staff with pertinent information at onset of office hours.

Responsabilités

• Maintains open communication as admin on-call coordinator with all clients, physicians, staff and referral sources.
• Follow up on all unresolved issues from office hours (i.e. lab results).
• Coordinates new patient starts as required.
• Documents communications, referrals, and problems as appropriate and in a timely manner.
• Provide office staff with all pertinent information at onset of office hours.
