Job summary

Columbia River Home Health is seeking a licensed Occupational Therapist (OT) to join our patient-centered team in Washington. This role offers a full-time, part-time, or PRN path with a Monday–Friday schedule and no nights or weekends.

You’ll provide skilled in-home OT services, develop individualized care plans with the interdisciplinary team, and educate patients and caregivers on adaptive techniques and exercises.

Qualifications

• Washington OT license required or able to obtain before start date.
• 1–2 years OT experience; home health preferred.
• Current CPR certification.
• Valid driver’s license with reliable transportation.
• Strong communication, critical thinking, and time management skills.

Responsibilities

• Provide skilled OT services in patients' homes.
• Assess patients’ functional abilities and develop individualized care plans with the interdisciplinary team.
• Instruct patients and caregivers on adaptive techniques, energy conservation, and home exercise programs.
• Complete thorough evaluations, progress updates, and discharge documentation accurately and on time.
• Support safe discharge planning and facilitate transitions across care settings.

What You’ll Do

What We’re Looking For

• Licensed to practice as a Occupational Therapist (OT) in Washington State (or able to obtain before start date)

• One to two years of experience in occupational therapy; home health experience preferred

• Current CPR certification

• Valid driver’s license, reliable transportation, and auto insurance

• Strong communication, critical thinking, and time management skills
