Job summary

Sweetwater Care is seeking a licensed Occupational Therapist to evaluate, treat, and counsel patients recovering from illnesses and injuries. The role develops individualized treatment plans, conducts therapeutic activities, and collaborates with the care team to optimize mobility and pain management.

You will document progress, adjust plans per outcomes, and provide discharge summaries in compliance with regulations.

Qualifications

• Requires Master’s degree in Occupational Therapy and active OT licensure.
• Proven ability to assess patient condition and develop treatment plans.
• Experience in documenting progress and communicating with care teams.

Responsibilities

• Consult with patients to assess condition and goals.
• Develop individualized treatment plans and conduct therapy sessions.
• Coach patients in exercises and therapeutic techniques.
• Maintain accurate patient records and progress notes.
• Provide discharge summaries per regulatory requirements.
• Supervise assistants as needed and participate in trainings.
