Job summary

Carilion Clinic is seeking an Occupational Therapist for Home Health in Virginia. The role involves assessing patients, interpreting referrals, and designing individualized treatment plans tailored to developmental/chronological age and medical diagnoses.

You will coordinate care with families and the care team, and document progress in patients’ records. The position requires a Virginia license, basic life support, and strong communication and teamwork.

Qualifications

• Education: Bachelor's or Master's degree in Occupational Therapy from an accredited program.
• Experience: One year of clinical experience preferred for Home Health, not required.
• Licensure: Licensed to practice as an Occupational Therapist in Virginia.

Responsibilities

• Assess and interpret physician referrals and develop individualized treatment plans.
• Coordinate patient care with families, therapists, and medical team.
• Document in medical records and comply with regulatory requirements.
• Supervise OTAs and collaborate with staff to ensure quality patient care.
• Engage in program development, orientation, and committee activities as needed.
