Qualifications

• Current Occupational Therapy licensure in the state of Washington.
• Current CPR certification.
• Current driver's license and vehicle insurance or access to reliable transportation.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provides service within the scope of practice as defined by WA state laws, per plan of care, with evidence-based techniques, coordinating with the health care team.
• Evaluates the patient's functional status and CMS-required status; consults with the physician to develop the therapy plan of care.
• Observes, records, and reports patient responses to treatment and changes to condition to nurse supervisor and physician.
• Conducts patient assessments evaluating functional level using appropriate tests.
