Tri County Home Health in Texas seeks a PRN Occupational Therapist to deliver direct patient care to homebound clients. You will develop and adjust treatment plans, monitor patient progress, and collaborate with other health professionals to ensure comprehensive care.

You will also supervise on-site therapists, educate families, document care in progress notes, and protect patient confidentiality while evaluating outcomes.

Qualifications

• Current Texas OT license.
• Current certification in BLS/CPR.
• Valid Texas driver's license and auto insurance.
• Knowledge of Home Care Home Base software is a plus.

Responsabilités

• Develops and revises plan of care in consultation with physician and other care-team members.
• Provides therapeutic treatments and evaluation of equipment needs as applicable.
• Participates in clinical conferences and in-service programs.
• Conducts on-site supervisory visits of certified occupational therapy assistants.
• Advises and consults with the family and other agency personnel as applicable.
• Prepares clinical and progress notes.
• Protects and honors patient and coworker confidentiality and right to privacy.
• Evaluates outcomes of care.
