• Maintains patient records and charts to ensure accurate treatment records.
• Identifies OT needs to achieve long- and short-term goals.
• Participates in Continuous Quality Improvement Program.
• Performs other duties as assigned.

Job Summary

Organizes and conducts medically prescribed therapy programs in the home/daycare center to restore function to patient.

Required

• Education: Undergrad degree
• Experience: 1 year, one to two (1-2) years of experience in occupational therapy
• Licenses and Certifications: BLS (Basic Life Support) certification, course accredited by the American Heart Association (AHA) or American Red Cross (ARC) upon hire; Valid Texas Driver's License, proof of driver’s insurance upon hire; Current licensure as Occupational Therapist upon hire

• Experience: Home Health experience
