Qualifications

• Licensure as an Occupational Therapist in the practicing state.
• ACOTE-accredited OT education and NBCOT certification requirements.
• Eligibility or completion of NBCOT entry-level exam.

Responsibilities

• Implement occupational therapy services per physician's plan of treatment.
• Assist evaluation of client functioning and contribute to care plan revision.
• Provide therapeutic activities to restore function and self-care.
• Observe, record, and report client responses to treatment and changes.
• Educate client/caregiver and health team on OT phases and adaptive devices.
• Provide staff in-service education as directed.
• Assess and reevaluate client needs; revise care plan as needed.
• Initiate diagnostic, preventive and rehabilitative procedures as appropriate.
• Maintain continuity of multidisciplinary care with the team.
• Promote care coordination through conferences and physician/field staff communication.
• Prepare and maintain individualized care progress records with accuracy.
• Stay current on OT trends for service provision and documentation.
• Participate in staff meetings, in-service programs and reviews.
• Contribute to quality improvement and supervise OT Aide monthly.
