Job summary

All About Home Care Inc. in Houston area seeks an Occupational Therapist (OT) to serve patients in the Upper Central North Greater Houston area, including Conroe, Spring, and The Woodlands.

You will perform initial evaluations, develop plans of care, and deliver therapy in patients' homes, using Kinnser software for documentation. The role requires Texas licensure, an accredited OT degree, and experience in home health preferred.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AMA/AOTA and/or certified by passing the national Certification exam.
• Currently licensed in the State of Texas.
• Two years of experience in Home Health preferred.
• 1 year experience using Kinnser/WellSky for home health.

Responsibilities

• OT takes initial history and makes initial evaluation.
• Assists the physician in evaluating level of function.
• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive living within the patient's capabilities.
• Provides prescribed occupational therapy.
• Directs and supervises personnel as required.
• Performs all skilled procedures.
• Consults with physicians regarding change in treatment.
• Writes reports to physicians regarding patient's progress.
• Instructs patient/family members in home programs and fine motor development exercises.
• Initial evaluation, plan of treatment and goals submitted for physician approval; progress notes each visit; re-evaluation when duration ends; discharge summary.

All About Home Care is a Houston-based home health care agency. We've been serving the Houston and surrounding communities since 2004, providing quality home health services. We are currently looking for Occupational Therapists OTs to help serve patients in the Upper Central North of the Greater Houston area, which mainly includes Conroe, Spring, The Woodlands, and surrounding areas. Qualification

• Graduate of an Occupational Therapy curriculum accredited jointly by the committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association (AOTA) and/or certified by successfully completing the national Certification examination
• Currently licensed in the State of Texas
• Two years of experience in Home Health preferred
• 1 year experience using Kinnser/WellSky for home health

• An initial evaluation, including plan of treatment and goals, must be filled out and submitted to the physician for approval. A progress note is written each visit. A re-evaluation is conducted when expected duration of treatment is reached. A summary is written upon patient's discharge.
• Helps develop the plan of care and revises as necessary
• Prepares clinical and progress notes
• Advises and consults with the family and other Agency personnel
• Periodically participates with all other health care personnel in patient care planning
• Participates in in-service program
