Job summary

Acute Home Healthcare is seeking a Home Health Occupational Therapist (OT) to support patients across Orange, Osceola, and Seminole counties. The OT will provide in-home therapy, design patient-centered plans, and collaborate with a multidisciplinary team to achieve functional goals.

This role offers flexible scheduling, near-home visits, and online documentation, with supportive management and 24/7 clinical support. Orlando relocation is required prior to starting work.

Qualifications

• Two years or more of home health experience required.
• Currently licensed in the state where you practice.
• Level 2 background screening required (as linked).

Responsibilities

• Improves or minimizes residual physical disabilities of the patient.
• Returns the individual to optimum and productive living within the patient’s capabilities.
• Participates in patient care planning with other health care personnel.
• Provides prescribed occupational therapy.
• Takes initial history and makes initial evaluation.
• Consults with physicians regarding changes in treatment.
• Writes reports to physicians regarding patient’s progress.
• Instructs patients and family in home exercise programs and activities.
• Assists physician in evaluating function level and revising plans of care.
• Prepares clinical and progress notes.

Qualifications of Home Health Occupational Therapist (OT):

• Currently licensed in the state(s) in which practicing
• At least 2 year or more of experience in home care

• Takes initial history and makes initial evaluation.
• Consults with physicians regarding change in treatment.
• Writes reports to physicians regarding patient's progress.
• Instructs patients/family members in home programs and fine motor movement exercises.
• Assists the physician in evaluating level of function.
• Helps develop the plan of care and revises as necessary.
• Prepares clinical and progress notes.
• Advises and consults with the family and other Agency personnel.
• Participates in Inservice programs.
