Job summary

Hope Home Care - a LiveWell Partners Network Agency in the United States is seeking a compassionate Occupational Therapist to work one-on-one with patients in their home environment, developing and overseeing occupational care plans.

As an OT you will evaluate patient needs, create treatment plans, provide therapy services, and coordinate with physicians and the care team to ensure continuity of care. The role involves field work with a Monday–Friday schedule and on-the-road duties.

Qualifications

• Minimum of 1 year of occupational therapy experience with a preference for 1+ years in an acute or a post-acute care setting.
• Active state licensure in Occupational Therapy is required.
• Proof of current CPR certification and valid driver’s license.
• Proof of current auto insurance coverage.

Responsibilities

• Establish patient goals and help patients meet their unique goals by providing quality care, including assessing and interpreting evaluations and test results.
• Determine occupational therapy treatment plans in consultation with the care team and physicians.
• Administer occupational therapy treatments as ordered by a physician and direct treatment given by assistants.
• Evaluate effects of occupational therapy treatments; recommend adjustments and modifications.
• Maintain equipment and work area in a safe and clean condition.
• Complete discharge planning by consulting with the care team and physicians and assure any appropriate continuation of the therapeutic plan.
• Participate in and contribute to case conferences.
• Document patient care services by charting in patient and department records.

Compensation

Guaranteed base Salary, Per point incentive pay in addition to the base salary, Per point value between $61 and $67, Potential earnings of $3,654 - $4,038 bi-weekly and above, Payroll processed bi-weekly (26 pay periods), Mileage reimbursed at $0.55/mile.

Shift Type

Full-time, Monday - Friday, 8 AM to 4 PM (flexible), In the field/On the road at 15-30% of the average 40 hour work week.

Summary

We are looking for a compassionate Occupational Therapist (OT) to work one-on-one with our patients in their home environment to establish and oversee patients' occupational care needs.The Occupational Therapist (“OT”) is responsible for evaluating/assessing the needs of our patients and formulating treatment plans in the home care environment.
