Elara Caring is seeking an Occupational Therapist for a staff position in Cherry Valley, Massachusetts. The role offers an hourly rate of $41–$58, with duties focused on evaluating patients, developing and implementing care plans, and coordinating with the care team.

The candidate should have an accredited OT program, an active OT license in the state, at least 1 year of clinical experience, and willingness to travel up to 50% within the coverage area.

Competenze

• Graduate of an accredited Occupational Therapy program.
• Current, unrestricted OT license valid for the state of work.
• Minimum one year of experience in a clinical care setting.
• Able and willing to travel within branch/office coverage area (50%).

Mansioni

• Evaluates the occupational therapy patient to determine rehabilitation needs and develops a plan of care.
• Implements and develops the Plan of Care to ensure quality and continuity of care and proper discharge planning; conducts initial and follow-up evaluations.
• Communicates significant findings and changes to the supervisor, physician, and care team.
• Instructs patients and families in activities of daily living and home programs, using adaptive equipment as needed.
• Supervises and provides direction to COTA/HHA and staff to ensure the care plan is followed.
