The VNA Health Group seeks a full-time Occupational Therapist to provide home health OT evaluations and treatments across Middlesex County, NJ. You will assessment clients, plan therapies with physicians, and document outcomes while supporting caregivers and families.

Qualifications

• NJ Occupational Therapy licensure required.
• 2–3 years of clinical experience; community-based experience preferred.
• Strong computer skills and communication/interpersonal abilities.
• Valid NJ driver’s license.

Responsibilities

• Support organization’s mission by striving for excellence in all aspects of their job with a focus on positive interpersonal relationship with co-workers.
• Assess and evaluate clients for occupational therapy needs and develops and implements appropriate plan of action in collaboration with the physician.
• Document all changes in client’s condition and updates care plans and outcomes accordingly.
• Identify patient, caregiver, and family counseling and education needs and plans appropriate teaching strategies.
• Monitor quality and appropriateness of services provided.
• Participate in VNAHG’s QAPI and in-service programs.
• Adhere to agency infection control and safety policies.
• Adhere to the organization’s policy in regard to absenteeism and appearance.
