Job summary

BAYADA Home Health Care in Raleigh, NC, is seeking an experienced part-time Occupational Therapist for home health visits in our Raleigh-Durham Senior Living office. You will join a multidisciplinary team to deliver rehabilitative care so clients can remain at home.

Responsibilities include evaluating function, planning interventions, and educating patients and families. Prior home care experience is preferred; NC OT license is required.

Qualifications

• Current North Carolina OT license.
• Graduation from an ACOTE-accredited OT program.
• Minimum one year of recent OT experience.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; EMR experience preferred.

Responsibilities

• Make home visits to clients within designated geographic territories.
• Perform thorough evaluations of functional status and ADL/IADL performance.
• Assess cognitive, psychological and motor aspects to plan care.
• Evaluate home environment for hazards and independence barriers.
• Identify equipment needs and environmental adaptations.
• Revise care plan and coordinate with nursing on the multidisciplinary team.
• Educate clients and family members in rehabilitation and safety.
• Document observations and interventions using a touch pad tablet.
