Job summary

Health Wealth Safe, Inc. is seeking an Occupational Therapist (OT) to assess, teach, and deliver therapy interventions within home health. You will collaborate with interdisciplinary teams to optimize patient well-being through assessments, goal setting, and treatment planning.

Responsibilities include conducting evaluations, administering tests, and preparing plans of care. You will supervise OT Assistants and Home Health Aides where applicable, and complete OASIS documentation where permitted

Qualifications

• OT degree from an accredited program.
• Current, unrestricted OT license.
• CPR certification is required.
• Preferred: strong organization, communication, attention to detail, time management, and experience with home health therapy and OASIS documentation.

• Administer and interpret tests to establish functional goals and outcomes.
• Prepare written evaluations and plans of care, including progress and discharge plans.

• Maintain plans of care, modify goals as needed, and collaborate on discharge planning and patient education.
• Coordinate with healthcare team members, families, and caregivers.
• Supervise related personnel such as OT Assistants and Home Health Aides where applicable.
• Complete assessments, including OASIS documentation where permitted by regulation.
