Job summary

AgeRight Care at Home is seeking a licensed Occupational Therapist for our Home Health team in Oregon. You will develop treatment plans and deliver home-based therapy. Educate patients and families, complete notes, and adjust plans to meet goals.

Requires Oregon OT licensure and CPR; home health experience preferred. We offer flexible scheduling and growth opportunities, with a starting pay range of $45–$56 per hour plus benefits: mileage reimbursement, up to $25,000 for education, PTO, 401K

Qualifications

• Must be licensed as an Occupational Therapist in Oregon.
• Graduate from an accredited program with a degree in Occupational Therapy.
• CPR Certification is required.
• NPI number required upon hire.

Responsibilities

• Establish treatment plan appropriate to physician orders.
• Prepare/complete clinical and progress notes.
• Teach patient and family/caregivers in meeting individual treatment goals.
