Homeside Rehab in New York seeks a compassionate Occupational Therapist or Occupational Therapy Assistant to join our home health team. You will evaluate patients, develop individualized plans, and deliver evidence-based OT services in patients' homes.

You will collaborate with a multidisciplinary team, document outcomes, educate families, travel to patients’ homes within the service area, and adapt approaches for diverse diagnoses.

Competenze

• Active NYS OT license (OT or COTA) required.
• Bachelor's/Master's/Doctoral degree in OT or OTA program completion.
• Home health experience preferred; new graduates welcome.
• Proficient in electronic documentation; valid driver's license.

Mansioni

• Evaluate patients and develop individualized treatment plans based on assessment and goals.
• Provide evidence-based OT interventions to improve motor skills and daily living activities.
• Maintain accurate documentation of patient care per regulatory requirements.
• Collaborate with multidisciplinary clinical team for coordinated treatment.
• Communicate progress and care plans with office staff, patients, and families.
• Educate patients and families on strategies to sustain progress between sessions.
• Adapt treatment approaches for diverse diagnoses, ages, and functional levels.
• Travel to patients' homes within the service area and maintain a reliable schedule.
