Adoration Health is seeking a dedicated Home Health Occupational Therapist to provide patient-centered OT services in Concord, NC. The role focuses on 1:1 care, independence, and collaboration with a multidisciplinary team to support patients in a home health setting.

The position offers a weekdays schedule (8a-5p), opportunities for growth, and a supportive work environment within a growing organization committed to patient dignity and quality home-based care.

Formación

• Graduate of a CAPTE-approved Occupational Therapy Education program.
• Current state OT licensure and American Heart Association CPR certification required (may be offered upon hire).
• Two years of experience in adult occupational therapy; home health experience preferred.
• Excellent observation, verbal and written communication, problem solving, and basic math skills.

Responsabilidades

• Perform occupational therapy evaluation and treatment per physician orders, including recommendation and procurement of adaptive equipment.
• Establish measurable goals and develop treatment plans to move patients toward independence and/or function.
• Maintain communication among the treatment team, physician, patient, and family members as applicable.
• Maintain thorough and timely documentation of services provided to ensure proper billing.
• Supervise Occupational Therapy Assistants in direct patient care and patient-related activities in accordance with state practice act.
