Job summary

Qualifications

• Current OT license to practice in the assigned state.
• Graduate of ACOTE-accredited OT program.
• Eligible to take the NBCOT certification exam.
• One year related OT experience; mentoring if less.

Responsibilities

• Deliver OT services under physician-approved plan of care.
• Ensure plan of care addresses identified problems.
• Maintain timely and accurate documentation.
• Supervise home health aide when RN not active.
• Perform other duties as assigned.
