Job summary

Elite Home Health, part of LHC Group, seeks an Occupational Therapist in Home Health to assess and treat patients in their homes, focusing on functional status and activities of daily living. The OT will coordinate with the healthcare team and physicians to develop the plan of care based on medical conditions.

Qualifications

• Current Occupational Therapy licensure in state of practice.
• CPR certification required.

Responsibilities

• Provides OT services within scope of practice per state laws and plan of care.
• Evaluates patient functional status and documents CMS-required data; coordinates with physician.
• Observes and reports patient responses to treatment to nurse supervisor and physician.
• Conducts assessments to evaluate functional ability using appropriate tests.
