Job summary

Optimal Care is seeking a Home Health Occupational Therapist to work one-on-one with patients in their homes across the Bay County service area, traveling daily from Saginaw to residences. You will assess functional abilities and guide patients as they regain independence in daily activities.

You will design personalized treatment plans, educate families, and collaborate with a multidisciplinary team to support safe, meaningful living.

Qualifications

• Graduate of an ACOTE-accredited occupational therapy program.
• Current state license as an Occupational Therapist.
• Minimum 1 year of experience as a licensed Occupational Therapist.
• Reliable transportation, valid driver’s license, and valid automobile insurance.
• Willingness to travel daily for home visits within service area.

About the Role

Help patients regain independence and return to the daily activities that bring meaning to their lives. As a Home Health Occupational Therapist, you'll work one-on-one with patients in their homes, using creativity, clinical expertise, and problem-solving to help them rebuild the skills needed for everyday living. From dressing and bathing to cooking, managing routines, and engaging in meaningful activities, you'll guide patients as they rediscover their ability to live safely and independently.

This role is ideal for a licensed Occupational Therapist who thrives in a flexible, patient-centered environment and enjoys tailoring care to each individual's unique goals and home setting. You'll assess functional abilities, design personalized treatment plans, and develop adaptive strategies and solutions that support independence and safety. Working closely with patients, families, and an interdisciplinary care team, you'll provide education, hands‑on training, and encouragement as patients progress toward greater confidence and independence in their daily lives.

Location

• Office Location: Saginaw
• Main Service Area: Bay County, Midland, Clare, Gladwin, Arenac
• This role requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

• Full Time, Monday to Friday, 8 a.m. to 5 p.m.

Required Qualifications

• Graduate of an accredited occupational therapy program (ACOTE accredited)
• Current state license as an Occupational Therapist
• Minimum 1 year of experience as a licensed Occupational Therapist
• Reliable transportation, valid driver's license, and valid automobile insurance coverage
• Willingness to travel daily for home visits within service area

Preferred Qualifications

• Home health experience
• Experience with adaptive equipment and home modifications
• Knowledge of Medicare home health documentation

Pay Range

$84,000—$89,000 USD
