Job summary

Optimal Care is seeking a Home Health Occupational Therapist to help patients regain independence in daily living tasks, such as dressing, bathing, cooking, and routines, in their homes across the Jackson area.

The role involves one-on-one care, developing personalized treatment plans, and educating patients and families while collaborating with an interdisciplinary team. Travel daily within the service area is required, with hours Monday–Friday and occasional weekend rotation.

Qualifications

• Graduate of an ACOTE-accredited occupational therapy program.
• Current state licensure as an Occupational Therapist.
• Minimum 1 year of licensed OT experience.
• Reliable transportation, valid driver's license, and automobile insurance.
• Willingness to travel daily for home visits within service area.

Responsibilities

• Assess functional abilities and design personalized treatment plans.
• Provide hands-on training and education to patients and families.
• Deliver care in patients' homes and coordinate with an interdisciplinary team.

About the Role

Help patients regain independence and return to the daily activities that bring meaning to their lives. As a Home Health Occupational Therapist, you'll work one-on-one with patients in their homes, using creativity, clinical expertise, and problem-solving to help them rebuild the skills needed for everyday living. From dressing and bathing to cooking, managing routines, and engaging in meaningful activities, you'll guide patients as they rediscover their ability to live safely and independently. This role is ideal for a licensed Occupational Therapist who thrives in a flexible, patient-centered environment and enjoys tailoring care to each individual's unique goals and home setting. You'll assess functional abilities, design personalized treatment plans, and develop adaptive strategies and solutions that support independence and safety. Working closely with patients, families, and an interdisciplinary care team, you'll provide education, hands‑on training, and encouragement as patients progress toward greater confidence and independence in their daily lives.

• Office Location: 801 Rosehill Road, Jackson, MI 49202
• Main Service Area Options:
  • Lansing: Eaton / Ingham
  • Central: Jackson / Washtenaw / Livingston
  • South: Hillsdale / Lenawee / Monroe
• This role is a high travel position and requires daily travel within the service area to provide care to patients in their homes, assisted living facilities, or nursing homes

Hours

• Monday to Friday, 8:00 AM - 5:00 PM, with weekend rotation

Required Qualification

• Graduate of an accredited occupational therapy program (ACOTE accredited)
• Current state license as an Occupational Therapist
• Minimum 1 year of experience as a licensed Occupational Therapist
• Reliable transportation, valid driver's license, and valid automobile insurance coverage
• Willingness to travel daily for home visits within service area

Preferred Qualifications

• Home health experience
• Experience with adaptive equipment and home modifications
• Knowledge of Medicare home health documentation

Pay Range

$84,000—$89,000 USD
