Optimal Care is seeking a licensed Occupational Therapist to provide home health services in Jackson, MI. You will assess patients in their homes, design personalized treatment plans, and help individuals regain independence in activities of daily living.
Travel daily within the service area, collaborate with families and an interdisciplinary team, and educate patients on adaptive strategies and safe home practices.

As a Home Health Occupational Therapist, you'll work one-on-one with patients in their homes, using creativity, clinical expertise, and problem-solving to help them rebuild the skills needed for everyday living. From dressing and bathing to cooking, managing routines, and engaging in meaningful activities, you'll guide patients as they rediscover their ability to live safely and independently.
